Prepare the article type, patient-consent statement, clinical-image files and author declarations, then choose one of the three approved submission routes.
Confirm fit before opening the system
The submission should center on a documented human clinical case or related cases and explain its diagnostic, management, procedural, safety or educational contribution. Clinical Images may carry that contribution through an image-led clinical account. Interdisciplinary work is eligible when the patient record remains central. Diagnostic challenges and unusual treatment responses are themes within the four accepted formats, not additional categories.

Prepare the complete submission record
Manuscript
An editable Word file containing the title page, abstract, main text, references, tables and figure legends.
Clinical images
Separate high-resolution TIFF, JPEG or PNG files, labeled consistently and accompanied by explanatory legends.
Authorship
Names, affiliations, corresponding-author details, contributions and ORCID iDs where available.
Declarations
Patient consent, ethics information where required, competing interests, funding, data availability and prior dissemination.
A concise cover letter should identify the article type, clinical learning point, scope fit, consent status and any related manuscript, preprint or conference abstract. For clinical video or moving images, confirm supported file formats and publication arrangements with the editorial office before submission; video is a medium, not a separate article type.
Confirm before submission
- All listed authors have approved the manuscript and its submission to JCCI.
- The work is original, sources are attributed, and the manuscript is not under consideration by another journal.
- Disclose related manuscripts, preprints, conference abstracts and other prior dissemination in the cover letter, with relevant links or copies.
- Disclose material AI use, including the tool and its purpose. Authors remain responsible for accuracy, originality, attribution and confidentiality; AI tools cannot be authors.
Patient consent, privacy and ethics
- Include the written consent-for-publication statement required for the case details and images.
- Remove direct identifiers from the manuscript, figures and embedded image metadata before upload.
- Supply ethics committee or institutional review board information where the work falls within its remit.
- Confirm permission for reused or adapted figures and cite the original source.
- If consent cannot be obtained, explain the circumstances to the editorial office before proceeding. Any exception requires assessment under journal policy, applicable law and institutional requirements. De-identification alone does not grant permission to publish.
The manuscript supplied to reviewers must contain the consent statement, not the signed consent form. Signed consent material is handled only by authorized editorial personnel through the agreed confidential channel and must not be circulated to reviewers or included in public supplementary files.

What happens after submission
- Technical check
- The editorial office checks file completeness, consent and ethics statements, declarations and essential formatting.
- Editorial screen
- An editor assesses article type, scope fit, clinical relevance, image readiness and whether the conclusions match the evidence.
- Peer review
- Peer review: Single-blind by default; double-blind review is available on request. Manuscripts that proceed to external peer review are normally evaluated by at least two independent subject-matter experts. Revision may be requested before a decision.
- After acceptance
- The accepted record proceeds to invoicing, copyediting, typesetting, author proof review and publication preparation.
Fees are separate from the decision
There is no submission fee. The applicable article processing charge is invoiced only after editorial acceptance; rejected manuscripts do not incur an APC. Fee status, payment and waiver requests do not influence peer review or the editorial decision.

Withdrawal policy
Authors should contact the editorial office promptly and wait for written confirmation before submitting the work elsewhere.
Manuscripts withdrawn within 3 days of submission are not subject to a withdrawal charge.
Manuscripts withdrawn more than 3 days after submission incur a withdrawal charge equal to 30% of the journal's APC.
Authors must receive official withdrawal confirmation before submitting the manuscript to another journal.
